nursing for patients who no longer require immediate access to the
full range of services provided in an acute hospital but who are
not yet ready for final discharge.
The patients in a "virtual ward" of
the hospital remain the clinical responsibility of their hospital
consultants. You have immediate access through MediHome's web
accessible EPR system to all the latest observations and
documentation (including images if required) from MediHome Nurses,
retain control over your patients' treatment, and have confidence
in MediHome's robust clinical governance.

GPs
Patients receiving care from
MediHome Nurses in a "virtual ward" operated for your local
hospital have not yet been discharged and remain the clinical
responsibility of the hospital clinicians. MediHome provides
a 24 hour on call service to support its patients, protecting you
from any additional workload. MediHome works closely with
the hospital and PCT and liaises closely with GPs on final
discharge to ensure a seamless handover.
Patients receiving care from
MediHome Nurses as a result of PCT commissioning to avoid hospital
admissions remain the clinical responsibility of their GPs unless
the PCT has made other arrangements. As the the responsible
clinician you can manage your patient via the EPR system.

Private Practice
MediHome Nurses have proved
popular with private patients and their consultants since we
starting offering the service in 2003. As a consultant
treating patients privately, you can manage the continuing care of
your patients through the EPR system and have confidence in the
