Mrs B, a 45 year old
woman with 2 children and no immediate access to 24 hour child
care, contacted her GP following 3 days of pain, swelling and
redness in her left lower leg. Otherwise she was well,
subject to monitoring by her GP for possible raised hypertension.
She had returned from an overseas holiday ten days previously,
reinforcing a diagnosis of DVT resulting from her flight.
The GP referred Mrs
B to MediHome. The MediHome Nurse arranged for phlebotomy
and delivered a 5 day course of Clexane injections under the
oversight of the GP. Mrs B attended her local acute hospital
for outpatient ultrasound and X ray tests. Further blood
samples were taken in the patients' home by the MediHome Nurse who
arranged for delivery to the laboratory. The MediHome Nurse
visited twice daily, administering Clexane and phlebotomy in the
morning and updating the warfarin dosage in the evening under direction from the
laboratory and the GP.
MediHome Nurses
visits ceased after six days once the warfarin dosage had
stabilised.
