case study 4 - aortic valve replacement

Wheelchair bound for six months due to severe breathlessness secondary to a diseased aortic valve, other significant medical history of Mr A included insulin dependent diabetes, polio affecting his left leg (therefore, poor mobility), obesity, iron deficiency anaemia and severe concentric left ventricular hypertrophy. Mr A stayed in hospital for two weeks following his aortic valve replacement; this included 48 hours in Intensive Treatment Unit in the early post operative phase. He walked for the first time on day 13 following surgery and was discharged into MediHome care on day 15 with urinary catheter in situ.

MediHome care included assistance mobilising, monitoring diabetes, regular blood tests, ECGs, cardiac assessments and provision of daily reports to the cardiologist. After progressive, intensive rehabilitation and step down in care in liaison with doctor, Medihome services were able to be withdrawn on the fourth week following surgery and Mr A was cared for thereafter by local district nurses.
