job description for nurse (evenings/weekends)

 

Responsible to:     Clinical Care Co-ordinator
 

Accountable to:    Director of Nursing

 

Normal Shift Patterns:

                          07.00 - 15.00

                          15.00 - 23.00

 

Salary:                     £25,000 - £31,000 p.a plus unsocial hours

                          (Unsocial hours supplements apply

                          20.00 - 23.00 and to weekend working)

 

Job Summary:

Actively participate in, supervise and maintain the highest level

of care for patients.

 

Contribute to the general management of MediHome through

membership of both the clinical and business teams.

 

Contribute to raising the profile of MediHome and thereby

developing new business for MediHome.

 

The role includes evening, morning and weekend work and

participating in an On-Call rota.

 

Principal Accountabilities:

Deliver at all times high quality nursing care by assessment

implementation and evaluation of patients' needs.

 

Undertake patient visits (scheduled and unscheduled) during

the evenings and weekends.

 

Maintain and support harmonious working relationships within

MediHome.

 

Act in such a manner as to satisfy public trust and confidence,

to uphold and enhance the good standing and reputation of

the nursing profession and MediHome Ltd.

 

Undertake the On Call duties including liason with hospital

sites.

 

Maintain accurate documentation at all times adhering to NMC

standards.

 

Assess patient care needs and plan episodes of care, including

nurse visits out of hours.

 

Establish and promote sound lines of communication to all

staff, patients and hospitals.

 

Supervise the work for junior colleagues.

 

Work with the medical staff in order to ensure positive

outcomes for each patient episode.

 

Formulate, in conjunction with senior staff, procedures and

standards, and to participate in clinical and organisational

audit.

 

Responsible for ensuring adequate equipment/supplies to

undertake patient visits.

 

Participate in staff appraisal and the identification of

professional needs. Set and evaluate personal objectives.

 

Participate through mentorship in the orientation and

development of staff.

 

Participate in "in-house" training sessions and attend

meetings and discussions when required.

 

Utilise all resources within MediHome effectively to deliver care

at all times.

 

Actively involved in nursing research projects in support of

clinical practice.

 

General

Be personally accountable for own actions and omissions in line

with the NMC Code of Professional Conduct and to take every

opportunity to maintain and improve knowledge and

professional competence.

 

Ensure compliance with all MediHome corporate policies, local

policies and legal requirements relating to clinical care and

Health and Safety, and the storage of drugs.

 

Ensure that equipment is handled correctly and maintained in

safe working order.

 

Maintain and develop a harmonious working relationship within

MediHome and with other external agencies.

 

Ensure that MediHome confidential information is not disclosed,

including information about staff and patients, to any

unauthorised member of staff, or to anyone outside the

employment of MediHome who is not authorised to such

information.

 

Promote the interests of MediHome and encourage the

development of new business links and associations.

 

This job description is intended to reflect, in outline, the

envisaged responsibilities of the post-holder, which may

change over time with the corporate and local business needs

of MediHome Ltd. It will be subject to periodic review, as a

R.G.N. 1st Level

Evidence of continuing professional development

ENB 998/ C&G 730 or equivalent

ENB community course and/or equivalent experience

     

Knowledge & Experience

At least 3 years post registration experience

Current experience with cannulation and venepuncture

Experience of working on own

Ability to communicate well, verbally and in writing

Ability to work well as part of the team and be flexible

Previous community care experience and experience in an acute care environment (ITU, A&E etc.)

Wound care and associated products experience

Current experience with IV treatment/therapy

Skills & Abilities

Ability to demonstrate own initiative thinking

Flexibility

Able to cope with ambiguity

Excellent communication / interpersonal skills

Time management skills

 

Personal Exprience

Able to work independently and carry out assigned tasks in a changing demanding environment

Previous experience at managing your own case load independently
