MediHome works with PCTs to complement
the services already provided by community matrons and district
nurses. We are also pleased to work with PCT provider
units to help design services that draw on MediHome's expertise
and systems delivering acute nursing in the community in
partnership with existing community nursing teams.
Our services to PCTs fall into
four categories:
-
Facilitating discharge of
patients incurring excess bed days charges in the local acute
hospital, who are off the critical list but still require acute
nursing.
-
Providing an alternative to
admission for patients requiring a variety of intravenous
therapies, including common antibiotics and rehydration.
-
Providing support to patients in
the community who would otherwise be admitted for a short period
of nursing and observation, generally in a Medical Assessment
Unit. Some may be subsequently admitted for more intense
treatment, but most are discharged from care by a MediHome Nurse
as they respond to treatment and their condition becomes less
acute.
-
Delivery of intravenous therapies
in the community including chemotherapy for cancer
patients and drugs for patients with conditions such as
haemophilia and MS.
MediHome's services are generally
substantially less expensive than treatment in hospital and more
attractive to the patient.
