If you are being
cared for by MediHome following a hospital visit, you will be
assessed by a MediHome Nurse either before you go to hospital or
whilst you are there. In any event, shortly after arriving home
from hospital, you will be visited by one of our nurses.
Alternatively, for certain conditions a doctor or community matron
may arrange for a MediHome Nurse to visit without you ever having
to visit the hospital.
The MediHome Nurse
will check you are settled in at home and that everything has been
done to ensure that you have a comfortable and trouble free
recovery. The MediHome Nurse will then administer any necessary treatments
and medications, as prescribed. They will also be happy to answer
any questions and give you any advice you need.
We can also provide
additional services. For example, it may be that you require a
physiotherapist to visit you in your home, in which case we will
arrange this.
MediHome uses an
advanced
electronic patient record system that your doctor is able
to view over a password protected secure internet connection.
Your MediHome Nurse will either phone through her notes to our
head office for entry onto the system, or enter them
directly using a pocket sized wireless computer.
The MediHome Nurse
will leave you with a folder containing everything you need to
know about the management of your condition and the MediHome
service.
The MediHome
information folder will be contain a telephone number to call in
the event that you have any problems. This number is manned 24
hours a day.
